Hospice Of The Chesapeake Launches Chesapeake Palliative Medicine
Hospice of the Chesapeake announced recently the official launch of Chesapeake Palliative Medicine in Annapolis, Maryland. Chesapeake Palliative Medicine will bring together physicians and nurse practitioners who specialize in the interdisciplinary care of individuals living with chronic complex illness.
The Chesapeake Palliative Medicine team will work directly with patients to define goals of treatment, navigate the healthcare system, and define their own values during illness as well as provide practical support for the family. The palliative plan of care is integrated into every stage of the illness along with all other appropriate medical treatments.
Palliative care consultations will be available for those patients living with the pain and symptoms of heart disease, COPD, cancer, liver or kidney failure, neurological conditions and complications from surgery.
